Personal Health Assessment

Our SOS members are entitled once each year to receive a very comprehensive health evaluation by a qualified physician for about the same price you'd pay for a dinner entree at your favorite restaurant.

You'll find the Personal Health Assessment survey below. If you're not an SOS member but would like to receive a personal health evaluation for minimal cost, you're welcome to go to the Log On link and enroll in SOS now.

The difference between our Health Assessment and one you would receive from your doctor is not just the price. It's the evaluation. Our physician promotes nutrition as the key to your good health. He will not prescribe a drug to treat your symptoms. He will provide expert guidance on how to restore health by using nutrition, including additions to your diet, avoiding toxins that cause your symptoms, and the use of supplementation when necessary with effective products.

To correctly complete the PHA survey, check the box next to any symptom that has manifested, or for any diagnosed condition you're aware of.

When you finish a section, you'll see a total number of points on the line that says "Your Score" at the end of each section. The doctor will use this point system to evaluate your personal health status, so please take your time completing the form. Be sure to read each section carefully so you don't overlook something. Each item is important to an accurate evaluation and, if applicable, a recommendation for the appropriate remedy to get you back on track toward good health. Some questions are asked more than once. Be sure to answer the question each time it is asked. Allow yourself about 60 minutes to complete the 974 question survey.
